What is Ocular Motility Testing

Ocular Motility Testing is a type of eye examination that is used to assess the movement of the eyes.It helps to identify a range of eye conditions that affect the ability of the eyes to move and focus accurately.

The test involves the patient following a light or object with their eyes while the doctor watches the eye movements.Eye movements are assessed for accuracy, speed, range of motion and ability to follow the object accurately.

Ocular motility testing can help diagnose conditions such as strabismus, nystagmus and convergence insufficiency.The test is completely non-invasive and painless and does not require any special preparation.

When to do Ocular Motility Testing

  1. Ocular Motility Testing should be done in people with vision complaints, especially those with double or blurred vision, people with a history of a head or eye injury, or those with neurological or motor movement disorders. It should also be done in children and adults with a family history of strabismus (crossed eyes).
  2. A person should consult a doctor urgently if they experience sudden double or blurred vision, dizziness, or blackouts. They should also contact a doctor immediately if they have a severe headache, confusion, or a sudden change in their vision. Other urgent situations include a sudden eye injury or a noticeable difference in the movement of the eyes.

  1. Strabismus: Ocular motility testing can help diagnose strabismus by assessing the patient's ability to move their eyes in different directions and how well their eyes move together in tandem.
  2. Nystagmus: Ocular motility testing can help diagnose nystagmus by assessing the patient's ability to move their eyes in different directions and how well their eyes move together in tandem.
  3. Optic Neuritis: Ocular motility testing can help diagnose optic neuritis by testing the patient's ability to track objects with their eyes. It can also help to determine if there is any evidence of diplopia (double vision) or an afferent visual defect (lack of vision in one eye).
  4. Ocular Myasthenia Gravis: Ocular motility testing can help diagnose ocular myasthenia gravis by testing the patient's ability to move their eyes in different directions and how well their eyes move together in tandem.
  5. Multiple Sclerosis: Ocular motility testing can help diagnose multiple sclerosis by testing the patient's ability to track objects with their eyes. It can also help to determine if there is any evidence of diplopia (double vision) or an afferent visual defect (lack of vision in one eye).
  6. Uveitis: Ocular motility testing can help diagnose uveitis by testing the patient's ability to move their eyes in different directions and how well their eyes move together in tandem.
  7. Graves' Disease: Ocular motility testing can help diagnose Graves' Disease by testing the patient's ability to move their eyes in different directions and how well their eyes move together in tandem.
  8. Traumatic Brain Injury: Ocular motility testing can help diagnose traumatic brain injury by testing the patient's ability to track objects with their eyes. It can also help to determine if there is any evidence of diplopia (double vision) or an afferent visual defect (lack of vision in one eye).
  9. Glaucoma: Ocular motility testing can help diagnose glaucoma by testing the patient's ability to track objects with their eyes. It can also help to determine if there is any evidence of a relative afferent pupillary defect (RAPD), which can indicate the presence of glaucoma.
  10. Age-Related Macular Degeneration: Ocular motility testing can help diagnose age-related macular degeneration by testing the patient's ability to track objects with their eyes. It can also help to determine if there is any evidence of a central scotoma (blind spot), which can indicate the presence of AMD.

Symptoms that require Ocular Motility Testing

  1. Diplopia (double vision): You may experience seeing two or more images of a single object, or the inability for your eyes to align and focus on the same object.
  2. Ptosis (drooping eyelid): You may experience one or both of your eyelids drooping or sagging.
  3. Strabismus (eye misalignment): You may experience your eyes not aligning properly and focusing on different objects.
  4. Nystagmus (involuntary eye movements): You may experience an involuntary and repetitive movement of your eyes.
  5. Ocular Flutter (rapid eye movements): You may experience rapid movements of your eyes that are uncontrollable.
  6. Ocular Aligning Reflex (inability to align eyes): You may experience difficulty with getting your eyes to align properly, even when looking at the same object.
  7. Ocular Misalignment (eyes not pointing in the same direction): You may experience one or both of your eyes not pointing in the same direction.
  8. Diplopia in Far and Near Vision: You may experience double vision in both short and long distances.
  9. Abnormal Pursuit (inability to track objects): You may experience difficulty with tracking objects with your eyes.
  10. Oculomotor Dysfunction (inability to move eyes): You may experience difficulty with moving your eyes in different directions.

How Ocular Motility Testing is done

  1. Ocular motility testing is a procedure that measures the ability of your eyes to move together in a coordinated fashion. This test is used to diagnose a variety of eye conditions, such as strabismus (crossed eyes) and nystagmus (involuntary eye movements).
  2. The test is performed using fully automated equipment. The equipment will track the movements of your eyes while you look at a target. The target may be a single point or a series of points. As you look at the target, you will be asked to move your eyes in different directions. The equipment will measure and record the speed, accuracy, and coordination of your eye movements.
  3. At the end of the test, the results will be interpreted by your doctor to determine if there is any evidence of an eye condition. Depending on the results, your doctor may refer you for further testing or treatment.
